When you fill out the application completely and submit it you will receive
a free meal on your birthday and a 10% discount each time you visit any
of our locations.
INSTRUCTIONS:

of age or older to apply.
2. One application per person.
3. Complete the application.
4. Your permanent card will be sent directly to your home.

Free birthday meal in not valid with any other discount, offer, promotion,
the Ful Boat at Ivar's Seafood Bars, entree items of $20 or more in Ivar's
full serve restaurant, or alcoholic beverages or gratuties. Ivar's reserves
the right to ask for posistive indentification or to alter or cancel this
offer at any time. The free birthday meal expires 14 days after the date
on the card.
